This survey is designed for individuals living with HIV/HCV/STBBIs in Canada who have participated in or may be interested in accessing rehabilitation and wellness services/programs at community-based HIV organizations.
Realize would love to hear about your experiences, preferences and needs accessing these services and programs at community-based HIV/HCV/STBBI organizations.
This anonymous survey will help Realize develop educational resources and training tools for front-line service providers in community-based HIV/HCV/STBBI organizations to help them plan, and integrate rehabilitation and wellness programming into their support services
This anonymous survey should take no more than 15 minutes to complete, and will be available until April 13, 2018.
