Job Summary:
Under the direct supervision of the Manager of Westshore AVI Health Centre, or designate, the Westshore Systems Navigator is responsible for the delivery of support, advocacy, harm reduction and outreach services to individuals accessing the Westshore AVI Health Centre. They will work as part of a multidisciplinary health team to provide a broad range of psychosocial and health-related supports to individuals in accordance with the clinic’s goals and objectives. This role is key to the clinic’s goal of keeping people engaged in care and maximizing treatment outcomes.

Job Summary:
The SAFER Outreach Support Worker applies their lived/living experience of illicit drug use to build and maintains relationships with people who use drugs, specifically with those individuals who are supported by the SAFER Initiative. They will practice harm reduction in their relationships to build trust and provide community peer support. They will work as part of the SAFER team, which includes nurses and physicians, to increase accessibility to pharmaceutical alternatives to toxic drugs and to respectful, non-judgmental health care. The SAFER Outreach Support Worker will be integral to building stronger community connections with people who face ongoing health and social barriers such as poverty, homelessness, and criminalization.

Job Summary:
The Victoria SAFER Initiative is a flexible, community-based, safe supply model with health care provider oversight. Its goal is to save and affirm the lives of people who use substances by providing safer, pharmaceutical alternatives to the currently toxic supply.
The SAFER Outreach Registered and Licensed Practical Nurse will work in partnership with an interdisciplinary team to provide participant-centered healthcare including assessment, screening, and education to support participants who are accessing the SAFER Initiative. The SAFER Outreach Registered and Licensed Practical Nurse will use a harm reduction, trauma-informed approach and is required to have demonstrated clinical skills and experience to independently assess individuals with complex primary care health needs and active poly-substance use.
