Position: HIV/HCV Case Manager
Wage: Based on Experience
Location: 40 Begbie Street, New Westminster, BC
Hours: 1 FTE, Mon – Fri business hours, evening and weekends as required. 35 hours per week.

Stride with Purpose provides supports and services for people living with HIV, HCV and/or people who use drugs, under a community health model. The Case Manager role collaborates and acts as an extended member of the HIV clinical case management team to perform a wide variety of supportive tasks to assist in the prevention of the progression of HIV and promote overall optimal health for their client base. The HIV/HCV Case Manager will prioritize and collaborate on client care plans deemed as high priority based on clinical factors assessed by the Fraser Health Sexual Health team; while simultaneously providing ongoing support to maintain clients’ engagement in care. Communication, collaboration and supporting developed care plans with the Sexual Health team is an essential function of this role.
Core Duties:
- Provide health and wellness support as well as crisis intervention services through low barrier and easily accessible services
- Provide 1-on-1 case management, including assistance securing and sustaining emergency and/or long-term housing, income supports, nutritional supports, peer-based supports and other community supports.
- Facilitate and supervise drop-in services 3x per week for clients, including the provision of wellness-focused programming, weekly food bank and community lunch
- Assist with organizing appointments, travel and related accompaniments/outreach activities in partnership with the Sexual Health/BBI Team
- Update, consult and provide feedback to Sexual Health/BBI team on important factors related to client engagement and disengagement from community supports.
Additional Duties:
- Maintain strong knowledge on core competencies and diversified models of care for targeted populations
- Attending relevant meetings, labs, and conferences with a direct focus on HIV, HCV, sexual health, drug use, mental health, poverty, and stigma issues.
- Maintain confidential and accurate documentation of client interventions
- Other duties as required
Qualifications:
- A 4 year degree from a recognized post-secondary institution in Social Work, Human Services, Health Sciences, or other related programs. A relevant two year diploma plus extensive experience in a related field may also be considered.
- Experience providing services for at-risk and/or marginalized populations.
- Experience in a non-profit organization is an asset.
- Ability to function effectively in difficult and/or crisis situations is required.
- Knowledge of local resources is an asset.
- Clear criminal records check.
