Over the past two years, weāve worked hard to develop a barrier-free, wraparound care plan that includes:
ā¢ On-site point-of-care testing and blood draws
ā¢ Referrals and doctor collaboration
ā¢ Medication prescriptions
ā¢ Ongoing medication adherence support via local pharmacies and our clinic
