Position Summary:
The Health Promotion Case Manager, working within a harm reduction model, will use an intensive case management model to work collaboratively with clients to decrease vulnerability and risk for acquiring HIV while increasing their resiliency and self efficacy through the development of a personalized resiliency plan. The Case Manager will work with individuals from one of the following identified key populations: other men who have sex with men (OMSM); others who have additional risk factors such as recent diagnosis/treatment of sexually transmitted infection(s), being part of a serodiscordant couple, participating in the sex trade; experiencing mental health and/or substance use issues; homelessness or inadequate housing. Over a time-limited period of 6-8 visits per episode of care with a follow up at 3-6 months, the Case Manager conducts an intake, uses a standardized risk assessment process to address health status, financial/legal issues, housing status, transportation, social support, nutritional health and mental health/addictions, and develops a personalized resiliency plan. This position reports to the Health Promotion and Community Case Management Supervisor.
