Case Manager – TAHAH (Towards Aboriginal Health and Healing)
Temporary Maternity Leave – Starting December 2012
Position Summary:

The TAHAH Case Manager works out of the Vancouver Native Health Society with AIDS Vancouver and the VCH STOP team within the TAHAH project.  The primary focus of this position is to increase access to medical and social services for Aboriginal men and women who are particularly isolated from key HIV services and who present with advanced HIV and multiple barriers to care.  Working within a harm reduction model, Case Managers deliver services to agency clients by supporting the development of personalized health care plans which contain, as appropriate, strategies for improved medical support, housing, income, psycho-social needs and individual capacities as well as avenues to establish linkages with other community resources.  This position reports to the Case Management Supervisor – Offsite.
