Job Posting: Outreach/Case Manager, Heart of Richmond

Job Details: This is a part time position – approx. 21 hours per week (Tuesday to Friday – Flexible hours)

Compensation: Starting at $26/hr, Salary will depend on the applicant’s qualifications, experiences, and skillset.

 

We are seeking an individual with an experienced background in Social Work and/or Psychiatry with a knowledge of HIV. The position best suits an out-going personality, as it is mainly to provide support to members, their families and caregivers.  We are looking for an individual that can work independently as well as in a team environment and is able to promote education and awareness throughout the community.  The job description can sometimes change due to COVID-19 and current restrictions, therefore looking for an individual that can adapt quickly.

 

Key Role for Support Group/Members and Programs

  • Facilitate support group meetings (either in person or by Zoom meeting) and help coordinate distribution of food with the help of volunteers while following COVID-19 restrictions
  • Arrange guest speakers/videos on a variety of topics concerning the health and welfare for members
  • Works closely with the Gilwest Clinic to keep informed with the services and member referrals.
  • Maintain regular contact with members, providing support, advocacy, information and referrals as appropriate while abiding by COVID-19 restrictions.
  • Coordinate and maintain members eligibility of Grocery Access Program (GAP) and Supplementary Health Fund (SHF).
  • Arrange for members transportation to and from Hospital and other appointments (doctor, ie.) when needed.
  • Maintain up-to-date case reports and data for members along with recording event activities and attendance

Responsibilities for Education, Awareness & Prevention

  • Act as Community Liaison with other non-profits in the community.
  • Help with the execution of the HIV 101 presentations, while maintaining current material for prevention workshops and presentations
  • Solicits new opportunities for prevention workshops
  • Helps develop reporting and measurement practices to demonstrate program success.
  • Network with other AIDS organizations and Non profits, locally and nationally
  • Coordinates displays at various locations and events: BC AIDS Walk, Gay Pride Parade, World AIDS Day events, Kwantlen College, etc. (when current restrictions allow)
  • Helps coordinate the distribution of Society brochures/leaflets into the community along with coordinating new resource material for Safer Sex Campaigns/Condom packages.
  • Implement members programs funded by specific grants
  • Liaison with webmaster to keep relevant information up to date on the web site
  • Knowledge of Excel, PowerPoint and Social Media
